1. Purpose

This SOP governs COBRA continuation coverage administration for the Unity Care Member Plan Master Trust. Varipro serves as the COBRA Third-Party Administrator. This SOP defines qualifying events, notification responsibilities and timelines, election and payment procedures, monthly reconciliation, premium rates, and offboarding procedures.

2. Scope

Applies to all participating employer groups and their employees covered under the Unity Care Member Plan Master Trust. Covers all COBRA qualifying events, general notices, election notices, premium collection, and enrollment reconciliation with Allied Benefit Systems.

3. COBRA Administrator

COBRA Administrator: Varipro
Varipro handles all COBRA election notices, premium billing, payment processing, and enrollee communications on behalf of the plan. Operations is the primary contact between participating employers and Varipro.

Operations must notify Varipro of all new employer groups at onboarding and of all qualifying events within the timelines specified in Section 5. Varipro does not receive qualifying event notices directly from employers — all notifications route through Operations.

4. Qualifying Events

Disability Extension: If a qualified beneficiary is determined to be disabled (SSA) within the first 60 days of COBRA coverage, the 18-month period may be extended to 29 months. Varipro manages disability extension notices and premium adjustments.

5. Qualifying Event Notification Timeline

Step 1 — Employer Notifies Operations (within 30 days of qualifying event)

The participating employer (Plan Administrator) notifies Operations of any qualifying event within 30 days of the event date. Notification must include: employee name, event type, event date, coverage end date, and names of all affected qualified beneficiaries (including dependents).

Step 2 — Operations Notifies Varipro (within 14 days of employer notification)

Operations forwards qualifying event information to Varipro within 14 days of receiving notice from the employer. Operations uses Varipro's qualifying event notification form or online portal. Operations retains a copy of the notification and confirmation from Varipro.

Step 3 — Varipro Sends COBRA Election Notice (within 14 days of Operations notification)

Varipro mails a COBRA election notice to the qualified beneficiary at the last known address within 14 days of receiving Operations' notification. The election notice must include: coverage options, premium amounts (102% of applicable cost), election deadline, and payment instructions.

Step 4 — Qualified Beneficiary Election Window (60 days)

The qualified beneficiary has 60 days from the later of: (a) the date coverage was lost, or (b) the date the election notice was sent, to elect COBRA continuation coverage. Operations monitors election status via Varipro's reporting portal.

Step 5 — Premium Payment Window (45 days after election)

After electing COBRA, the qualified beneficiary has 45 days to submit the first premium payment (retroactive to coverage loss date). Subsequent premiums are due on the first of each month with a 30-day grace period. Varipro tracks payments and notifies Operations of delinquencies.

Step 6 — Enrollment Updated with Allied

Varipro transmits enrollment updates to Operations, who confirms that Allied's enrollment records reflect COBRA enrollees. Operations reconciles Varipro's active COBRA roster against Allied's enrollment data monthly (see Section 7).

Total Outer Deadline: The COBRA election notice must be sent no later than 44 days after the qualifying event (30-day employer notice + 14-day Operations-to-Varipro notice). Operations tracks this cumulative deadline, not just individual legs.

6. COBRA Premiums

COBRA premiums are set at 102% of the applicable cost of coverage (employer + employee share of premium, plus 2% administrative fee). Varipro calculates and bills COBRA premiums based on the plan option the qualified beneficiary elects to continue.

7. Monthly Reconciliation

Operations performs a monthly COBRA reconciliation on or before the 15th of each month:

Step 1 Operations pulls the Varipro active COBRA roster (all current COBRA enrollees by employer group, plan option, and coverage tier).
Step 2 Operations compares the Varipro roster to Allied's enrollment data for COBRA-coded members.
Step 3 Discrepancies (enrolled in Varipro but not Allied, or vice versa) are escalated to Allied and Varipro for correction within 5 business days.
Step 4 Operations tracks outstanding COBRA elections (notices sent, election not yet returned) and delinquent premiums (election made, payment overdue). A monthly summary is maintained in the Operations COBRA tracking log.
Step 5 Terminated COBRA (non-payment, end of coverage period, or new coverage obtained) is confirmed with Varipro and Allied enrollment is terminated accordingly.

8. General COBRA Notice (New Hires)

Per COBRA regulations, a general COBRA notice must be provided to newly covered employees and their spouses within 90 days of the date coverage begins. Varipro generates and mails the general COBRA notice for each employer group upon notification from Operations at onboarding. Operations confirms general notice distribution with Varipro at each new employer setup.

9. Employer Offboarding / Termination from Plan

All employees of a terminating employer trigger COBRA qualifying events upon the employer's plan termination date. Operations must notify Varipro at least 30 days before the employer's plan termination date to allow timely election notice generation.
Step 1 Operations receives notice of employer termination from employer or leadership at least 30 days in advance.
Step 2 Operations notifies Varipro of the employer termination date and provides a complete census of all covered employees and dependents.
Step 3 Varipro generates COBRA election notices for all affected employees and dependents with the termination date as the qualifying event date.
Step 4 Operations terminates the employer's enrollment in Allied. Existing COBRA elections continue through Allied until the maximum continuation period ends.
Step 5 Operations confirms with Varipro that all post-termination COBRA elections are processed and tracks through the normal monthly reconciliation cycle.
